As you may already know, we are conducting a communication assessment of the Austin History Center, and we would like to invite you to participate in this important effort. We hope to identify current communication patterns at the History Center, and then provide some concrete recommendations on what can be useful for improvement of communications and the use of new technologies for our communication.

Completion of the attached questionnaire, which begins on the next page, should take approximately 20-30 minutes. We shall consider your completed questionnaire as your consent to participate.

ALL INFORMATION OBTAINED IN THIS STUDY WILL BE CONFIDENTIAL.

All questionnaires will be returned to a specified mailbox at the Austin History Center and will be picked up daily by our student team at UT-Austin. Responses will not be identified with a specific individual. We will only report aggregate findings, and in no case will any individual names or information that in any way identifies specific respondents be reported. In addition to providing the History Center with a report of findings, we at the university may use this information in our ongoing research efforts to understand the role of communication in organizations. Again, your confidentiality will be protected at all stages of this project.

Your decision to participate or not participate in this assessment is voluntary, but we are truly interested in your feedback and hope you will be able to provide us with that. You may actually enjoy being able to voice your views on the issues covered in the questionnaire. If you decide to participate, you are free to discontinue your participation at any time. Choosing not to participate will not affect your future relations or standing with the History Center or The University of Texas at Austin.
