Obtain Approval
Discuss your choice of program, department, and field supervisor with the Capstone coordinator and faculty advisor, and have the Capstone coordinator contact your field supervisor.

Proposal

Submit all of the following documents to the Capstone coordinator:

Proposal
When approval of your assignment area and Field Supervisor has been obtained, develop a Professional Experience and Project Agreement Proposal (form: word)
Letter of Agreement
As a part of that proposal, prepare a Letter of Agreement for the signature of your Field Supervisor. The Letter of Agreement should be a modified form of the proposal language and should establish the details of objectives, hours, schedule, and methods and means of evaluation, plus whatever other language may be required by the Capstone field site. (example: word)

During Project

Field Trips
The faculty advisor should make at least two field trips to the project location during the duration of your program to talk with your field supervisor.

Evaluation of Student
The completed Student Practicum Evaluation form (word) needs to be signed by you and your field supervisor and given to your faculty advisor first. After all signatures are obtained, the original goes to the Capstone coordinator.
