Reservation Form for School of Information Equipment

The following form allows students at The School of Information to reserve items available for check out. Please note that this is a reservation only and only students currently enrolled in a School of Information course can check out equipment.
You will be required to fill out an official release form at the time you pick up the equipment. The check-out and check-in procedures may take several minutes. Please plan accordingly.
All equipment may be picked up after 2 PM on the first day reserved and is due by noon the day due. Equipment renewals are not always possible, due to high demand. In order to request a renewal, bring the equipment to the IT Lab and speak to an IT Lab staff member.

*Please note that the software available on the laptops is NOT identical to the software found in the IT Lab. All laptops have basic networking software, Bevoware software, open source applications, and Office applications. If you require a specific program, please speak with the IT staff.
